COVID Inside Visitation – Phase One
This plan is to assist in reducing the severity or seriousness of the spread of COVID-19 (SARS-COV-2) in the facility during the phase of reopening to resident visitation. At any time, even without prior notice, inside resident visitation may cease or be reduced to a lower phase. Center for Medicare and Medicaid Services (CMS) and Virginia Department of Health (VDH) continue to recommend outside (outdoors) visitation when possible.
Reasons that visitation would cease

We encourage our visitors (family & friends) to do their part in helping us keep the county positivity rate down and our residents safe by having good hand hygiene, social distancing, wearing a mask correctly in public, avoiding large crowds and getting vaccinated.
Process for phase one inside resident visitation (Visiting day)
This policy will not be altered in anyway without the written and signed approved from a corporate employee.

Reasons that visitation would cease
- If at any time the county or local positivity rate is greater than the current guidelines
- If at any time a resident test positive
- An outbreak of COVID in the facility is occurring
Phase One (1) COVID Inside Visitation Procedure
- All visits MUST be scheduled. Please contact the facility’s activity director or other designated person on the facility’s phone number.
- Visits will be limited to one (1) hour
- Limit of two (2) visitors at each visit
- Visit shall not occur at a meal time, unless it is a palliative/hospice visit
- There shall not be more than ten (10) visitors overall in the facility at one time.
- Visits will be on a “first come, first serve” basis and have limits to increase available times so all residents may receive a visit.
- Visits will occur in the resident’s room or designated visiting room with door shut.
- Residents that are in a private room – visits will occur in resident’s room
- Couples are considered as a private room (limit two (2) visitors per couple at a time)
- Residents in semi-private (companion) rooms – visits will occur in the designated visiting room
- Residents in a quarantine room - visits will occur in the quarantine room
- Residents that are in a private room – visits will occur in resident’s room
- At no time should other residents or other visitors be present during visits, except for any couples that resided in the facility.
- Visitors will be screened and temperature taken at the main door. If at any time the visitor has any symptoms or has a known exposure in the last 14 days; denial of visit will occur. Visitation will not be allowed at that time. We ask that if you know you have had a possible exposure or ill to please cancel your visit.
- All visitors at ALL times must wear an approved mask covering nose and mouth regardless of vaccination status. Mask should be snug fitting and stay in place when speaking. Face Shields, gaiters, and bandanas are NOT approved mask.
- Residents fully vaccinated are not required to wear a mask. Residents are encouraged to wear a mask. To be considered fully vaccinated you must have completed the vaccination series and two weeks have past.
- Residents not fully vaccinated will be required to wear a mask or a face shield during the entire visit.
- Visitors not following procedure set forth in the policy, may lose visitation privileges during phase one, as safety for the residents are our top priority.

Process for phase one inside resident visitation (Visiting day)
- Visitors should arrive on time for their scheduled visits. If you are early you may be asked to wait. If you are late, your visitation hour will still end one (1) hour after the scheduled time.
- Visitors are to park in the main parking lot and call the facility number to notify staff that you have arrived.
- Visitors are to remain in their vehicle. Visitors are to have mask on covering nose and mouth prior to the staff member approaching your vehicle. The mask MUST stay on at all times and always covering your nose and mouth, regardless of vaccination status.
- Visitors will be screened by the staff member that will come to your vehicle utilizing the recommended VDH symptom questions and have their temperature checked. Exhibit 2a
- If symptoms are present or temperature greater than 100.4F; entry and visit will be denied.
- If known exposure in the last 14 days, entry and visit will be denied.
- Visitors will sign in on the screening log
- Visitors will sanitize hands as soon as they enter the facility.
- Visitors will be escorted directly to the resident’s room or designated visiting room.
- At NO TIME should visitors be in the hallways except to walk to and from the resident’s or designated visitation room.
- At NO TIME should visitors be in the common areas of the facility.
- Enjoy your visit. We do encourage you to continue to social distance while visiting.
- When visitation is over, please activate the resident call bell and a staff member will escort you out of the facility using the main door. We request that you go straight from the resident’s or designated visiting room to the main door.
- Visitor will sign out on the screening log prior to exiting the facility.
